Who qualifies for the CCC Plus Waiver in Hampton Roads?
The Commonwealth Coordinated Care Plus (CCC Plus) Waiver serves people who are enrolled in Medicaid and meet nursing facility level of care, meaning they are functionally dependent with medical nursing needs, or who rely on technology and need substantial ongoing skilled nursing. They must also be at imminent risk of nursing facility placement and safe to serve at home.
It is a 1915(c) home and community-based services waiver, so it exists to keep people out of nursing homes. A widowed parent in Hampton who has Medicaid, needs hands-on help with most daily tasks and has a family willing to stay involved is the typical picture. Waiver coverage is for care at home; it does not pay assisted living room and board.
What services does the CCC Plus Waiver cover at home?
The waiver covers personal care, respite, adult day health care, a personal emergency response system with medication monitoring, assistive technology, environmental modifications, private duty nursing, services facilitation and transition services. Personal care and respite can be delivered by an agency or directed by the consumer, who hires their own aide.
Consumer direction suits families who already have a trusted caregiver, such as a neighbor or relative, but it makes the family an employer with paperwork to match. Agency direction is easier to start. Services facilitation helps consumer-directed families manage hiring and records. Adult day health care can double as respite for a caregiver working a shift at the shipyard or a Sentara unit.
What are the CCC Plus Waiver limits on hours and dollars?
The waiver caps personal care at 56 hours a week, with exceptions under set criteria, and respite at 480 hours per state fiscal year running July 1 to June 30. Environmental modifications and assistive technology are each limited to $5,000 per calendar year. Private duty nursing is capped at 112 hours a week.
Transition services, which help someone move from a nursing facility back into a home, are limited to $5,000 per lifetime. The table shows each limit. Fifty-six hours is a lot of care but not round-the-clock supervision, so a person with advanced dementia may still need other arrangements overnight.
| Service | Limit |
|---|---|
| Personal care | 56 hours a week (exceptions by criteria) |
| Respite | 480 hours per state fiscal year (July 1-June 30) |
| Environmental modifications | $5,000 per calendar year |
| Assistive technology | $5,000 per calendar year |
| Private duty nursing | 112 hours a week |
| Transition services | $5,000 per lifetime |
How do Hampton Roads families get a CCC Plus Waiver screening?
Screening starts with a long-term services and supports screening done by a local community-based team or, if your parent is hospitalized, a hospital screening team. Ask the hospital case manager at Sentara, Riverside or Bon Secours for the screening, or call your city's department of social services for the community team.
Medicaid enrollment is required too, so file that application in parallel rather than waiting. The screening produces the level-of-care finding that the waiver depends on, and it draws on the assessment described in our UAI guide. Then expect a wait for an aide or agency to be assigned. Plan for that gap with respite or private help.
Which PACE centers serve Hampton Roads, and where is the gap?
Three PACE (Program of All-Inclusive Care for the Elderly) centers serve Hampton Roads: two Sentara Senior Community Care PACE centers in Norfolk and Portsmouth, and InnovAge Virginia PACE in Newport News. PACE serves adults 55 or older who meet nursing facility level of care and can live safely in the community.
The Norfolk center, 757-252-7800, covers Virginia Beach and Norfolk ZIP codes. The Portsmouth center, 757-392-2650, covers Chesapeake, Suffolk and Portsmouth areas. InnovAge, 757-234-8100, covers Newport News, Hampton, Poquoson and lower York County. DMAS's ZIP lists show no PACE center serving Williamsburg, James City County, upper York County or Gloucester, so families there rely on the waiver or private pay.
How does PACE compare with the waiver, and where do we start?
PACE bundles Medicare and Medicaid benefits around a day center and care team, while the waiver pays for specific services chosen with a care coordinator. If your parent lives inside a PACE ZIP code, ask the center and the screening team how each would work for your parent, including which doctors and services each one uses, before choosing.
Start with your parent's city department of social services, which can begin the Medicaid application and the screening. Cardinal Care now manages most members' coverage, but the waiver kept its CCC Plus name.
